Andropause

Andropause affects most men after the age of 70. Some men in their 60s, and even some men in their 40s are affected.

Low testosterone can cause a decrease in memory, incentive, cognition, sexual interest, erections, ejaculation and more, but the most common symptom I see in our practice is depression. Many men in their 70s and older just sit around, have a beer, and watch T.V. It's sad.

Testosterone "normal" lab values are incorrect and indicate the usual values for that age patient. These values are usually 50% or even 75% less than what they should be. Also, measuring total testosterone and even "free testosterone" gives misleading information.

Giving testosterone to patients can be dangerous, as you know. Side effects can include accelerated prostate cancer, gynecomastia, increased prostatism, hair loss and even death, if not managed correctly. And giving testosterone to patients younger than 70 is misguided; it causes the hypothalamus to decrease pituitary output of luteinizing hormone, ultimately causing more testicular atrophy, which will not recover when and if the testosterone supplement is stopped. It also diminishes sperm production.
